• I didn't received the intake form till the day of my appointment and that was after I texted. I was not able to finish completing because I was in the class. Receptionist have to resend so I can complete that took away 15-20 min from my treatment session that wasn't professional. I did like the massage. Rosy was great therefore I bought the 6 treatments serial of one hour.
