A link was sent with the forms that I needed to complete. Could not find the link. It was not in the text I received nor the email. Could not sign into the patient portal I created when I scheduled the appointment. I was trying to get the link on my smartphone and it was very frustrating. Made me late for the appointment once I was handed the hard copy of the forms.
